Why This Question Matters

This episode opens with a big question: How much passive income is enough to walk away from medicine? Peter shares that it’s one of the most common questions he hears from doctors in the Passive Income MD community.

But instead of giving a simple answer, Peter always says: It depends. That’s because for most physicians, the real goal isn’t to leave medicine entirely, it’s to gain freedom. Freedom to work fewer hours, freedom to take breaks, and freedom to choose how medicine fits into their lives.

He explains that many doctors who reach financial freedom don’t actually quit. Instead, they reshape their work in a way that feels meaningful and sustainable. Passive income becomes a tool to create options, not an escape plan.

The key point here is that it’s not just about the money. The bigger question is about purpose, fulfillment, and designing a life that feels right. Passive income simply makes that possible.

Start With Your “Why”

Before you start calculating numbers, Peter encourages everyone to take a step back and ask: Why do I want passive income?

For some, the “why” might be more time with family, especially as kids grow. For others, it might be traveling more, doing mission work, or simply not working overnight shifts anymore. Everyone has a different motivation, and that motivation shapes your financial goal.

Peter explains that your reason is what drives the number you’re aiming for. Without clarity on your “why,” it’s easy to chase someone else’s idea of success and never feel satisfied.

He also points out that lifestyles vary. Some people live very simply. Others live in expensive cities, send kids to private school, or want luxury travel. All of these choices affect how much income you really need.

Essentials vs. Lifestyle

Once you know your “why,” Peter suggests breaking your income goal into two parts:

  1. Essentials (Peace of Mind Number): This is the amount needed to cover your basic expenses like housing, food, and transportation. It’s the safety net that ensures stability no matter what.
  2. Lifestyle (Ideal Life Number): This is the extra money needed for the things that make life richer, travel, private school, hobbies, or generous giving.

Peter says that for most people, the essentials number provides the peace of mind they’re really looking for. Interestingly, he’s found that when people finally calculate their numbers, they often discover their target is lower than they expected. That means financial freedom may be closer than they think.

He challenges listeners to spend just one hour going through their expenses and figuring out these numbers. It’s not glamorous work, but it’s the first step to creating clarity.

Reverse Engineering Your Freedom Number

Once you know your essentials and lifestyle numbers, the next step is building a plan. Peter suggests reverse engineering your way to financial freedom.

For example, if your goal is $10,000 per month in five years, then in year one, aim for $2,000 per month. In year two, build on that until you eventually reach your target. Breaking it down this way makes the goal feel doable instead of overwhelming.

Peter also points out that progress isn’t usually linear. It often starts slow but then picks up speed. As you learn more, build networks, and gain confidence, things move faster. He shares a story of someone who thought it would take 10 years to reach $20,000 per month in passive income but actually hit that goal in just three years.

Freedom, Not Escape

In the final section, Peter reframes the ultimate goal. It’s not about escaping medicine, it’s about creating choices. Passive income gives doctors the flexibility to practice in a way that feels good, while still having time and energy for life outside of work.

He makes it clear that he doesn’t want physicians to leave medicine altogether. Society needs them, and many still love their work. But he also acknowledges that today’s healthcare environment can make doctors feel exhausted or undervalued. Passive income creates the freedom to change that.

With financial freedom, physicians can rediscover the joy of medicine, cut out the parts that drain them, and live more balanced lives. The result is less burnout, more fulfillment, and the power to design both career and personal life intentionally.
