Medicine, which once promised freedom and opportunity, had narrowed his life instead, leaving little time for family, hobbies, or rest.

Peter acknowledges that burnout plays a big role. Many doctors feel trapped by shrinking autonomy and heavy administrative burdens. The message here is clear: success in medicine alone isn’t enough to guarantee fulfillment, and this realization is often the beginning of looking for something more.

Peter then shares his belief that expanding identity beyond medicine creates freedom. He points to a survey in his community where doctors who built new income streams and interests reported less stress and more job satisfaction. Even if informal, the results align with studies outside of medicine: diversifying identity leads to thriving.

He guides the audience through a short visualization exercise, asking them to imagine life five years from now. What would it feel like to travel more, build businesses, or spend more time with family? The point is to picture a future where “doctor” is just one part of who they are.

At the heart of this section is the phrase “I am.” Peter explains that what follows those two words shapes actions and life direction. This section dives deeper into how identity drives behavior. Peter mentions Robert Cialdini’s Influence, which shows that once people commit to an identity, their choices tend to align with it. Seeing yourself as an investor, for example, naturally leads you to seek opportunities and take action.

He illustrates this with a personal story about his daughter. In third grade, after struggling on a math test, she told him, “I am not good at math.” That identity shaped her behavior, she avoided math and gave up quickly. Instead of pushing harder drills, Peter focused on changing her self-belief. With small wins and encouragement, she eventually declared, “I am good at math,” and went on to succeed.

The story shows how powerful identity can be in either holding us back or propelling us forward. For physicians, shifting from “I’m just a doctor” to “I’m also an investor, entrepreneur, or learner” can unlock new doors. Growth starts with redefining who we believe we are.

Breaking Out of the Box

Peter talks about how limiting beliefs (like “just be a doctor”) keep people stuck. He recalls how his own father warned him against real estate investing because of a friend’s failure. But instead of listening, Peter attended a real estate conference under a fake business name. That first uncomfortable step eventually led to freedom and new opportunities.

He warns that the cost of not expanding can be greater than the risk of trying. Staying boxed in leads to lost autonomy, stagnation, missed financial chances, strained relationships, and the regret of unfulfilled potential. To make this real, he shares a chart marking the limited number of summers left with his daughter before college, a reminder of how precious time really is.

This section encourages listeners to see the hidden risks of inaction. By not stepping out, physicians risk losing far more than they might by failing at something new. The message: don’t wait until regret forces change.

The ACE Method

Peter closes with a simple framework: the ACE Method. “A” is for action; small steps build momentum and growth over time. “C” is for challenges; struggles are not obstacles but the very things that create strength. And “E” is for environment; surrounding yourself with the right people helps you grow into your new identity.

He reminds physicians of their capacity to learn by pointing to the Krebs cycle: if they once mastered complex science, they can also learn real estate or business. Growth comes through discomfort, and leaning into that discomfort is what expands identity.

Finally, he makes it interactive by having attendees write down a new identity on a “Hello, my name is” sticker, like investor, entrepreneur, or learner. He leaves them with this thought: being a physician doesn’t have to be your whole identity. You are “doctor plus,” and what comes after “I am” is yours to choose.
